Gene Therapy
Gene Therapy is getting popular from last decade. Some people are born with bad eyesight because they inherited from their parents’ mutated eye genes. Non invasive eye treatments (like eye exercises, Bates methods….) may offer little help because their eyes are programmed to be deteriorated. With invasive methods (laser surgery, eyeglasses), their eyes further affirm to go downhill as the method does not cure the root cause.

Genes, which are made of DNA, are the cell’s instruction manual, providing information on what molecules the cell should make and how the cell should act. The purpose of gene therapy is to try and change how the cell is acting by putting in a new gene or replacing a broken gene.

Common types of gene therapy:

  1. Gene Replacement (a new gene is introduced into a cell to replace a broken gene)
  2. Gene Editing (molecular scissors are used to “edit” a broken gene by cutting out the mutation)
  3. Gene therapy that stops cell death (stop retina cell from dying, so vision will not be lost)
  4. Gene Therapy to replace antii-vegf injections (Anti-VEGF injections are the most common treatment for Age Related Macular Degeneration and Diabetic Macular Edema, due to blood vessels in the retina grow uncontrolly and leak fluid into the eye. It slows down the growth of blood vessels)

  5. Gene therapy given through eyedrop – Case study: A patient has mutations in a gene that helps produce a protein called collagen 7, which holds together both skin and corneas. The treatment, called Vyjuvek, uses an inactivated herpes simplex virus to deliver working copies of that gene. The eyedrops use the same liquid as the skin version, just without the added gel.

  1. RPE65 gene is injected into the eye, under the retina because it is packaged into a safe virus called adeno-associated virus (AAV). The genetically modified AAV was able to significantly restore vision in blind children, enabling them to complete tasks such as walking through a maze without bumping into soft objects, or catching a ball.   3. LEPREL1 gene encodes an enzyme that is essential for the final modification of collagen in the eye. In the absence of the active form of this enzyme, aberrant collagen is formed, causing the human eyeball to be longer than normal. As a consequence, light beams entering the eyeball focus in front of the retina rather than on the retina itself and myopia emerges.

7. Hyperopia (Farsightedness)
• PDE11A plays an important role in conduction of signals for the management of eye growth. Tetratricopeptide repeat domain 30A (TTC3OA) protein and the alkylglycerone phosphate synthase (AGPS) are also involved in signal conduction between neurons along with PDE11A. The variant PDE11A is responsible for causing hyperopia.
• Cataracts develop by the mutation of AGPS, which is combined with a disorder of the multisystem development.
The MFRP gene helps to maintain the shape of the cornea and aids in the focusing mechanism. Mutation of this gene leads to farsightedness, retinitis pigmentosa, and micropthalamos.

• The PDGFRA gene binds to the receptor called platelet-derived growth factor (PDGF). PDGF is identified in the multiplying of retinal tissues, corneal epithelium, and it regulates the movement of corneal fibroblast. When the PDGFRA gene is arranged with three SNPs on chromosome 4q12, it results in the corneal bio-metrics leading to assurance of corneal astigmatism.
• The SHH (sonic hedgehog) gene plays a vital role in the development of several tissues, which also includes the eye. Studies have proclaimed that SHH gene is associated with irregular astigmatism.
• The VAX2 gene plays a major function in cornea development and increases the risk of astigmatism.

• Crystalline alpha A (CRYAA) comprises two genes: alpha-A and alpha-B. Mutations in these genes lead to congenital cataract and presbyopia.
• The protein concealed by a gene called RP1L1 along with retinal-specific protein plays a crucial role in altering the rod photoreceptors, thereby affecting the photosensitivity. Occult macular dystrophy is caused by the variation of this gene.

Invasive methods can solve patients’ eyesight issues quickly, but it only treats the symptoms, not the causes. Hence, eyesights usually continue to get worse along with time. Non invasive methods usually require more times, and the results are mixed, depending on the patients’ health, diet and living style.

Duke-NUS and NHCS scientists first in the world to regenerate diseased kidney

Blocking an immune-regulating protein reverses the damage caused by acute and chronic kidney disease, a preclinical study suggests.

They teamed up with scientists in Germany to investigate the role of IL-11, which is known to trigger scarring in other organs, including the liver, lungs and heart, in acute and chronic kidney disease.

Their findings implicate the protein in triggering a cascade of molecular processes in response to kidney injury that leads to inflammation, fibrosis (scarring) and loss of function. They also discovered that inhibiting IL-11 with a neutralising antibody can prevent and even reverse kidney damage in this setting.

“We found that IL-11 is detrimental to kidney function and triggers the development of chronic kidney disease,” said Professor Cook. “We also showed that anti-IL11 therapy can treat kidney failure, reverse established chronic kidney disease, and restore kidney function by promoting regeneration in mice, while being safe for long term use.”

More specifically, the researchers showed that renal tubular cells, which line the tiny tubes inside kidneys, release IL-11 in response to kidney damage. This turns on a signalling cascade that ultimately leads to increased expression of a gene, called Snail Family Transcriptional Repressor 1 (SNAI1), which arrests cellular growth and promotes kidney dysfunction.

In a preclinical model of human diabetic kidney disease, turning off this process by administering an antibody that binds to IL-11 led to proliferation of the kidney tubule cells and reversal of fibrosis and inflammation, resulting in the regeneration of the injured kidney and the restoration of renal function.

While clinical trials of an antibody that binds to another pro-fibrotic molecule called transforming growth factor beta have been unsuccessful, this new approach brings hope of a new target.

“By boosting the kidney’s intrinsic capability to regenerate, Prof Cook and Asst Prof Widjaja have shown that we can restore function to a damaged kidney,” said Prof Coffman, who is also the principal investigator of the Diabetes Study in Nephropathy and other Microvascular Complications (DYNAMO), a large collaborative study that aims to find new solutions for the prevention and treatment of diabetic kidney disease. “This discovery could be a real game-changer in the treatment of chronic kidney disease—which is a major public health concern in Singapore and globally—bringing us one step closer to delivering the benefits promised by regenerative medicine.”

please do a smart field on pudendal nerve decompression via laparoscopic surgery which attempt to remove muscle or tissue that is compressing the pudendal nerve. I have suffered the this pudendal nerve symptom for more than 8 years, most doctor and chiropractor doesn’t have any idea how to deal with this and i am at my wits end, please help.
